NDIS Support Coordination
Get the most from your NDIS plan with help from our support coordinators
If you receive Level 2 or Level 3 of Support Coordination for a primary or secondary psychosocial diagnosis, inform the NDIA you’d like to choose Stride as your NDIS provider and they will complete the referral to Stride.
Once we receive the NDIA referral, we will be in touch to organise the next steps.

Choose your support
As an NDIS support coordination provider, we offer two levels of support:
Our longer-term option. We’ll help you get to grips with your NDIS plan – working with you as you build your independence and relationships.
Specialist Support Coordination
Our highest level of support coordination. This is for people who have more complex needs, barriers to accessing supports and risks. Our Specialist Support Coordinators are registered Allied Health Practitioners and will assist you to manage any changes you may face.
