How to access support

Vibrant Futures is a voluntary, no cost service.

We accept self-referrals as well as referrals from GP’s, schools, government agencies and community organisations.

What to expect

Once your referral has been received, one of our Vibrant Futures team members will give you a call to discuss eligibility and ensure that we are best suited to support you and your families needs.

Our next available support worker will then get in contact to arrange a time for a home visit to meet, as well as have a chat about why you’re seeking support.

The support worker will discuss appointment times and locations and work with you to develop a Family Action Plan.
