Uplift is a youth mental health support program delivered by lead agency Stride and funded by the Darling Downs and West Moreton PHN to provide care to young people aged 12-25 years who are experiencing, or at risk of experiencing, moderate-to-severe and/ or complex mental health needs and functional impairment. Uplift is co-located with headspace Ipswich, supporting young people living in the West Moreton region.

About

Uplift provides a collaborative and integrative service to young people. Uplift consists of a multidisciplinary team that provides individual and group support based on the needs and goals of each young person.

Support includes evidence-based individualised psychological therapies, therapeutic case management, youth work, functional recovery, outreach support, access to medical care and access to education and employment services delivered by our onsite Individual Placement and Support (IPS) team.

What to expect

Uplift offers evidence-based mental health support that’s shaped around you and your goals. Our team works alongside you to improve wellbeing, build everyday life skills, and support study, work and personal goals. Care is practical, connected and focused on supporting your recovery.

For referrers

Referrals are accepted from GPs, community, service providers, school wellbeing staff, hospital and health services, as well as family and friends. Young people can also self-refer to Uplift Ipswich. On receipt of a referral, Uplift Ipswich will review the referral and contact the service provider to advise of outcome and then if applicable, will contact the young person to arrange an assessment.
